Nicole is considered an In-Network provider for United Healthcare/Optum. Nicole is happy to complete your medical billing at the time of service.

Nicole is not in network for any other other major insurance providers including BCBS or Cigna. Clients should expect to pay for sessions in full and are provided a monthly invoice with dates of service, fees and the proper medical codes needed for you to submit your insurance claims.

If you choose to use an insurance that Nicole is not currently in-network with, you may submit the invoice to your insurance company in order to be reimbursed a portion of the session fee. The amount you receive is different for each provider and individual plan. If you want to know exactly what amount you would expect to be reimbursed, you would call your customer service for your insurance provider. All policies, rates, and deductibles are different.

Questions to ask your Insurance Provider:

1. What are my “out-of-network, outpatient, mental health benefits” when seeing a licensed

    mental health counselor?
2. Do I need a referral from my primary care provider (PCP) to receive mental health services?
3. Do I have a deductible, coinsurance, or copay?
4. Have I met my deductible this year? When does my deductible restart?

5. Does pre-authorization apply? (Meaning: does the insurance company have to approve the treatment prior to             starting therapy?)

6. How do I file out-of-network claims?
